 Application forms for the admission to above Courses in Government,Grant-in-aid, Municipal and Govt. seats in Self financed Colleges after Qualifying Examination /12th Standard (Science Stream with B/AB Group) passed from the Gujarat Board, Central Board and Council of Indian School Certificate Examination for the academic year 2014-15 will be available only on payment of Rs.180/-(Rs.150/-Form Fee+Rs.30/-Processing Fee) in cash from the branches of Axis Bank approved by ACPMEC, (List of Axis Bank branchesis available on admission committee’s website) from06/06/2014 To   13/06/2014 during banking hours(Sundayclosed).

Duly filled application forms along with the required documents copies (Mark sheets, certificates, Caste Certificate, Non-Creamylayer certificate) will be accepted only at following centre inperson from 07/06/2014To   17/06/2014 between 10-00AM To 5-00PM (Process will be continue on Sunday). The application   forms of the eligible candidateswhohaveappearedinGUJCET-2014 only will be considered valid.

Candidates who want to take admission in Local Quota seats of  Smt. N.H.L. Municipal Medical College, Ahmedabad or SuratMunicipal Institute of Medical Education Research, (SMIMER) Surat, have to obtain the certificate of Local candidate from said colleges and attach the copy of the same with application form.

For physically handicapped candidates, to decide percentage of disability, decision taken by Board formed as per the Government Notification No.GP-11-MCG-1008-3544-J, dated 26-5-2009 shall be final. The candidate of   SEBC category will must attach the non- crèmelayer certificate of Competent Authority with application form.

Application   forms send by post, courier,   etc. will not be considered. Application forms received after 5-00pm on the last date i.e.,17/06/2014 will not be considered.
